Referring Doctors

Please prepare your patient by filling out our referral slip. A copy can be faxed to our office at 425-353-8517 and/or sent with your patient on the day of their consultation.  X-Rays can be emailed to hporalsurgery@gmail.com, or, if you are mailing films, please allow enough time for x-rays to arrive to our office prior to your patients appointment.
